Vitamin B12 deficiency does not present only with anaemia. Nervous-system symptoms such as numbness and tingling in the hands and feet, balance problems, forgetfulness, difficulty concentrating and tongue tenderness may also develop. In some people, neurological symptoms may occur before haemoglobin falls.
Groups that may require closer assessment for vitamin B12 deficiency include:

What Is the Relationship Between Vitamin B12 and Iron?
Vitamin B12 and iron are involved in different stages of red blood cell production. Both deficiencies may therefore lead to anaemia, although the laboratory patterns they cause may differ.
In iron deficiency, red blood cells usually tend to become smaller and MCV may fall. In vitamin B12 deficiency, red blood cells may become larger and MCV may rise. When both deficiencies are present in the same person, MCV may appear within the normal range because of these opposing effects.
A normal MCV therefore does not exclude vitamin B12 or iron deficiency on its own. Methylmalonic acid measurement may support diagnostic assessment when serum vitamin B12 is borderline.
What Is the Relationship Between Vitamin D and Iron?
Vitamin D is not considered the main vitamin that directly increases intestinal iron absorption. Vitamin C is one of the better-known dietary factors that more directly supports iron absorption. However, research suggests that vitamin D may have an indirect relationship with iron metabolism.
A regulator produced by the liver called hepcidin is central to this relationship. When hepcidin rises, the transfer of iron from intestinal cells and iron stores into the blood may decrease. Some cellular and human studies have shown that vitamin D may reduce hepcidin production. This mechanism may indirectly support the movement of iron into the circulation.
However, this result should not be interpreted as “My ferritin will definitely rise if I take vitamin D.” Vitamin D supplementation does not remove fundamental causes of iron deficiency such as heavy menstrual bleeding, gastrointestinal bleeding, celiac disease or insufficient dietary iron.
Does Vitamin D Deficiency Prevent Iron Absorption?
It is not accurate to say that vitamin D deficiency directly and completely prevents iron absorption. Although vitamin D and iron values may be related, a significant part of this relationship may involve inflammation, hepcidin, dietary patterns and accompanying conditions.
Not everyone with low vitamin D has low ferritin. Similarly, not everyone with low ferritin has vitamin D deficiency. When both values are low, the cause of each deficiency should be investigated separately.
Using only vitamin D in a person with iron deficiency does not replace iron treatment or assessment of the underlying cause. Likewise, iron supplementation does not correct low vitamin D that may be relevant to muscle pain and bone health.
Which Symptoms May Occur When All Three Are Deficient?
Because vitamin B12, vitamin D and iron deficiencies may cause certain shared symptoms, it is difficult to identify the low value based only on complaints.
Symptoms that may occur together include:

No single test explains the status of all three nutrients. Results should be assessed together with symptoms, medical history, diet and medicines.

Can Vitamin B12 and Iron Supplements Be Taken Together?
Because there is no major direct competition between vitamin B12 and iron absorption, many people may use both supplements on the same day. However, the form of the product, dose, gastrointestinal sensitivity and other medicines may change the appropriate timing.
The following points should be considered with iron supplements:
- They should not be taken with tea or coffee.
- They may need to be separated from calcium-containing supplements.
