What Do LDL, HDL and Triglycerides Mean?

LDL, HDL and triglyceride levels provide information about how fats are transported in the blood and the risks associated with cardiovascular health. High LDL may increase the risk of cholesterol accumulating in the artery walls. HDL helps transport cholesterol to the liver. Triglycerides are the type of fat in which the body stores excess energy that it does not use.

These values alone do not mean that “heart disease is present” or that “heart health is completely normal.” They should be assessed together with factors such as age, blood pressure, diabetes, smoking, kidney disease, family history and whether the person has previously had cardiovascular disease. The current approach recommends setting targets according to the person’s total cardiovascular risk rather than a single laboratory value.

What Do LDL, HDL and Triglycerides Mean in Brief?

The three main values in a lipid panel represent different functions and risks:

  • LDL cholesterol: It carries cholesterol from the liver and other tissues throughout the body. When the amount in the blood increases, the likelihood of cholesterol accumulating in the artery wall rises, which is why it is commonly called “bad cholesterol.”

  • HDL cholesterol: It contributes to transporting excess cholesterol in the blood to the liver. For this reason, it is known as “good cholesterol.” However, high HDL does not automatically eliminate the risk created by high LDL.

  • Triglycerides: They are the form of fat in which calories not immediately used by the body are stored. They are necessary for energy, but high levels in the blood may increase cardiovascular risk. Very high levels are also associated with a risk of inflammation of the pancreas.

LDL and HDL are related to lipoproteins that transport cholesterol in the blood. Triglycerides are a type of fat that is different from cholesterol. However, all three are assessed within the same lipid panel.

What Is a Lipid Panel?

A lipid panel or lipid profile is a blood test that measures different types of fat in the blood. It generally includes total cholesterol, LDL cholesterol, HDL cholesterol and triglyceride levels. Some reports may also include additional results such as non-HDL cholesterol and VLDL.

This test not only shows current values but also helps assess the person’s risk of atherosclerosis, heart attack and stroke. A lipid panel may also be used to monitor the effects of lifestyle changes or cholesterol-lowering treatment.

Cholesterol is not a completely harmful substance for the body. It is needed for the structure of cells, the production of certain hormones, vitamin D synthesis and substances involved in digestion. The problem is the long-term presence of high levels of lipoproteins that contribute to accumulation in the artery wall.

What Is LDL Cholesterol?

LDL stands for low-density lipoprotein. Its main function is to transport cholesterol through the blood to different tissues. The body needs this transport system, but when the amount of LDL increases, more cholesterol may be carried to the artery walls.

Over time, plaques made up of cholesterol, fat, calcium and other substances may develop in the artery walls. This process is called atherosclerosis. The growth of plaques may narrow the arteries, reduce blood flow and, in some situations, contribute to a heart attack or stroke.

When assessing an LDL result, the laboratory reference range is not the only factor considered. The LDL target for a person who has previously had a heart attack or stroke may not be the same as the target for a young person who does not smoke and has no additional risk factors.

What Is HDL Cholesterol?

HDL stands for high-density lipoprotein. It helps transport cholesterol from tissues and the circulation to the liver. The liver processes this cholesterol and contributes to its removal from the body.

For this reason, HDL is commonly described as “good cholesterol.” A low HDL level may be important for cardiovascular risk, particularly when it occurs with high LDL or high triglycerides.

However, a lipid panel should not be interpreted as “everything is fine if HDL is high.” A favourable HDL level does not erase the effects of high LDL on the arteries. LDL, non-HDL, triglycerides and the person’s overall risk profile are assessed together when making treatment and follow-up decisions.

What Are Triglycerides?

Triglycerides are one of the most common types of fat in the body. Energy obtained from food or produced from calories the body does not need is stored in fat cells in the form of triglycerides. These stores can be used when energy is required.

High triglycerides usually do not cause symptoms on their own. However, values that remain high for a long time may contribute to increased cardiovascular risk, particularly when they occur with high LDL, low HDL, diabetes, obesity or high blood pressure.

A triglyceride level above 500 mg/dL may increase the risk of inflammation of the pancreas, known as acute pancreatitis. Therefore, a very high triglyceride result should not be managed through dietary changes alone without medical assessment.

What Should LDL, HDL and Triglyceride Levels Be?

The reference ranges on laboratory reports may vary slightly depending on the method used. Personal targets may also differ according to the person’s cardiovascular risk. The ranges generally used for adults are as follows:

  • Total cholesterol: Below 200 mg/dL is generally considered desirable.

  • LDL cholesterol: Below 100 mg/dL is generally considered desirable for most adults.

  • HDL cholesterol: A level of 60 mg/dL or above is considered favourable. Levels below 40 mg/dL in men and below 50 mg/dL in women may be considered low.

  • Non-HDL cholesterol: Below 130 mg/dL is generally considered desirable for most adults.

  • Triglycerides: Below 150 mg/dL is normal, 150–199 mg/dL is borderline high, 200–499 mg/dL is high, and 500 mg/dL or above is considered very high.

These numbers help with general laboratory interpretation but do not represent the same treatment target for everyone. The current 2026 dyslipidaemia approach recommends lower LDL targets for people with a higher cardiovascular risk. In the assessment intended to prevent a first cardiovascular event, the LDL target may be below 100 mg/dL for people at borderline or intermediate risk and below 70 mg/dL for people at high risk. Lower targets may be set for people at very high risk who already have cardiovascular disease.

Therefore, an LDL level appearing within the laboratory reference range does not necessarily mean that the person has reached the target appropriate for their medical condition.

What Does High LDL Mean?

High LDL indicates that the risk of cholesterol accumulation in the artery walls and the development of atherosclerosis may be increased. This risk varies according to how high the LDL is, how long it has remained high and the person’s other medical characteristics.

Common causes of high LDL include a diet high in saturated fat, physical inactivity, excess weight, smoking, genetic susceptibility, diabetes, chronic kidney disease and hypothyroidism. Some medicines may also affect LDL levels.

An LDL level of 190 mg/dL or above is considered severe hypercholesterolaemia and requires medical assessment regardless of the person’s calculated risk. Such a result may suggest familial hypercholesterolaemia, particularly when there is a family history of a heart attack at a young age or very high cholesterol.

What Does Low HDL Indicate?

Low HDL indicates a lower level of the system that helps transport cholesterol from the tissues to the liver. It is not a diagnosis on its own but is assessed together with other risk factors.

Smoking, physical inactivity, excess weight, insulin resistance and high triglycerides may occur with low HDL. Rather than attempting only to increase HDL to a particular number, it is more important to focus on reducing overall cardiovascular risk.

Regular physical activity, stopping smoking, healthy weight management and a balanced diet may contribute to improvements in HDL and the overall lipid profile. However, a person should not start medicines or supplements without medical advice simply because HDL is low.

What Causes High Triglycerides?

High triglycerides may indicate that the energy the body receives or produces has increased in the blood and fat tissue in the form of fat. Excess calories, sugar and refined carbohydrate intake may particularly affect triglyceride levels.

Other conditions that may be associated with high triglycerides include:

  • Excess weight and obesity

  • Diabetes that is not under control

  • Hypothyroidism

  • Liver or kidney diseases

  • Physical inactivity

  • Excessive alcohol use

  • Certain medicines

  • Inherited lipid metabolism disorders

When high triglycerides are identified, not only dietary habits but also blood glucose, thyroid function, liver and kidney values, and current medicines may be reviewed.

How Are LDL, HDL and Triglycerides Interpreted Together?

The most accurate interpretation of a lipid panel is made by assessing the values together rather than independently. For example, when LDL is high, HDL is low and triglycerides are high, the overall picture may be less favourable in terms of atherosclerosis and metabolic risk.

When LDL appears normal but triglycerides are high and HDL is low, this may be associated with conditions such as insulin resistance, metabolic syndrome or type 2 diabetes. Looking only at the LDL result in this situation may not show the full risk.

Similarly, a high HDL level does not make high LDL or triglycerides unimportant. The CDC states that high triglycerides occurring with low HDL or high LDL may increase the risk of a heart attack.

The following questions should be considered when interpreting the test result:

  • Has the person previously had a heart attack, stroke or vascular disease?

  • Is diabetes, high blood pressure or kidney disease present?

  • Does the person smoke?

  • Is there a family history of heart disease at a young age?

  • Are the values high for the first time, or have they remained high for a long time?

  • Is the person taking cholesterol-lowering medicine?

A treatment decision should not be made according to a single laboratory value without this information.

Does High Cholesterol Cause Symptoms?

High LDL, total cholesterol and triglycerides often do not cause a clear complaint. A person may feel healthy while their lipid values are high. For this reason, cholesterol and triglyceride disorders are commonly detected during a blood test.

Common symptoms such as headache, fatigue, dizziness or neck pain should not be attributed only to high cholesterol. These complaints can occur in many different medical conditions.

The absence of symptoms does not mean that high cholesterol is unimportant. Risk may develop through the cumulative effect of the values on the arteries over many years. The interval for regular testing should be determined by a doctor according to the person’s age, family history and cardiovascular risks.

Is a Lipid Test Performed While Fasting?

The same preparation is not required for every cholesterol test. The doctor or laboratory may request that the test be carried out while fasting depending on the measurements required. When fasting is necessary, the person is generally asked not to consume food or drinks other than water for 9–12 hours before the test.

A repeat fasting test may be planned, particularly when high triglycerides are being investigated, a previous result was unexpectedly high or a treatment decision will be made. The person should not stop their medicines before the test and should follow the healthcare professional’s instructions about which medicines to take.

Alcohol consumption, a heavy meal, intense exercise, infection, pregnancy and certain medicines may affect the results. The conditions under which the test was performed should therefore be considered when interpreting it.

Which Other Values May Be Assessed in a Lipid Panel?

Current cardiovascular risk assessment is not limited to LDL, HDL and triglycerides. Other markers may be examined according to the person’s risk.

Non-HDL Cholesterol

Non-HDL cholesterol is calculated by subtracting HDL from total cholesterol. It includes LDL as well as other cholesterol-carrying particles that may contribute to atherosclerosis.

Particularly when triglycerides are high, the non-HDL result may provide additional information about the total atherogenic burden. A level below 130 mg/dL is generally considered desirable in adults, although the target varies according to personal risk.

Apolipoprotein B

Apolipoprotein B, or ApoB, may provide information about the number of lipoprotein particles that have the potential to accumulate in the artery wall. It may provide additional risk assessment in certain people whose particle number is high even though their LDL result appears normal.

The 2026 dyslipidaemia guideline states that ApoB testing may improve risk assessment in selected people with high triglycerides, metabolic syndrome or type 2 diabetes.

Lipoprotein(a)

Lipoprotein(a), or Lp(a), is a lipoprotein that is largely determined by genetic characteristics. A high level may increase the risk of cardiovascular disease.

The current 2026 approach recommends measuring Lp(a) at least once during adulthood. The result should be assessed together with other risk factors.

How Can LDL and Triglycerides Be Lowered?

The approach used to improve lipid values depends on which value is high and the person’s total risk. Lifestyle changes may be sufficient for some people, while medication may be required for others.

The general approach includes reducing saturated fat and processed food intake, increasing vegetables, fruit, whole grains and appropriate protein sources, regular physical activity, healthy weight management and stopping smoking.

Sugary drinks, excess refined carbohydrates and alcohol intake are particularly important in high triglycerides. People with very high triglycerides may need to stop alcohol completely and have their dietary plan arranged individually by a doctor and dietitian.

Cholesterol-lowering medicines are not started according to a laboratory result alone. The doctor assesses the person’s cardiovascular risk, other diseases, medicines and the expected benefit of treatment together. People taking medicine should also continue lifestyle measures such as a balanced diet, physical activity and stopping smoking.

Which Results Require Medical Assessment?

Not every high result in a lipid panel is an emergency. However, certain results require earlier and more detailed assessment.

Medical assessment should be obtained when LDL is 190 mg/dL or above, triglycerides are 500 mg/dL or above, values remain high on repeated tests or there is a family history of a heart attack or stroke at a young age.

When high triglycerides are accompanied by severe and persistent upper abdominal pain, pain spreading to the back, nausea or vomiting, rapid assessment is required for pancreatitis. Emergency healthcare should be sought regardless of the lipid result when there is chest pain, severe shortness of breath, cold sweating, facial drooping, speech difficulty or weakness on one side.

The initial assessment of lipid disorders may be carried out by Internal Medicine. People with high cardiovascular risk may be referred to Cardiology, while conditions associated with diabetes, obesity or thyroid disease may be assessed by Internal Medicine or Endocrinology.
