Can Sleep Apnea Be Detected During a Check-Up?

Sleep apnoea cannot be diagnosed with certainty through standard check-up tests. However, symptoms such as snoring, breathing pauses during sleep, daytime sleepiness and morning headaches, together with findings such as blood pressure, weight and neck circumference, may raise suspicion of sleep apnoea during a check-up.

For a definitive diagnosis, breathing, oxygen levels, heart rhythm and sleep structure must be assessed while the person is asleep. When suspicious findings are identified, laboratory polysomnography or a home sleep apnoea test in people considered suitable by a physician may therefore be planned.

From this perspective, a check-up is not a diagnostic method. It is the first assessment stage that helps identify risk and guide the person towards the appropriate investigation. Normal blood test results in a person with symptoms do not show that sleep apnoea is absent.

What Is Sleep Apnoea?

Sleep apnoea is a sleep disorder involving repeated pauses or significant reductions in breathing during sleep. These breathing interruptions may lower blood oxygen levels, repeatedly disrupt sleep and prevent the person from spending sufficient time in deep, restorative sleep.

The most common form is obstructive sleep apnoea. In this condition, the upper airway narrows or temporarily closes during sleep. In central sleep apnoea, the brain does not regularly send the signals needed by the breathing muscles. Both conditions require a sleep study and medical interpretation for a definitive assessment.

Sleep apnoea is not simply snoring. Not everyone who snores has sleep apnoea. However, more detailed assessment may be needed when loud, irregular snoring is accompanied by breathing pauses, waking while gasping or excessive daytime sleepiness.

How Is Sleep Apnoea Suspected During a Check-Up?

Standard check-up packages generally do not include a test that directly diagnoses sleep apnoea. However, risk may be identified by assessing symptoms, lifestyle, sleep habits and certain physical measurements during the check-up consultation.

Observations made by a partner or another person sleeping in the same room may be as important as the patient’s own report. The person may not notice breathing pauses or episodes of waking while gasping.

Assessment of Sleep Symptoms

A medical assessment may consider not only how many hours the person sleeps but also sleep quality and events occurring during the night.

The following concerns may be relevant to sleep apnoea:

  • Loud and irregular snoring
  • Breathing pauses noticed during sleep
  • Waking while choking or gasping
  • Frequent waking during the night
  • Dry mouth in the morning
  • Morning headaches
  • Severe daytime sleepiness
  • Dozing during meetings, while watching television or while driving
  • Difficulty concentrating and remembering
  • Irritability and mood changes
  • Frequently waking to urinate
  • Reduced sexual desire

People with sleep apnoea may not be aware of their night-time symptoms. Daytime sleepiness, fatigue, dry mouth, headaches and difficulty concentrating may be among the first concerns they notice.

Assessment of Physical Measurements

Weight, body mass index, waist circumference, blood pressure and neck circumference measured during a check-up may help assess sleep apnoea risk.

Excess weight and fat tissue around the neck may contribute to upper airway narrowing. However, sleep apnoea does not occur only in people with excess weight. Jaw structure, large tonsils, nasal obstruction and other anatomical features may also play a role in people with a normal weight.

Sleep apnoea should therefore not be diagnosed according to a single measurement. Physical findings, symptoms and medical history should be assessed together.

Assessment of Blood Pressure and Cardiac Findings

Some people with sleep apnoea may have difficult-to-control high blood pressure, night-time palpitations or heart rhythm problems. High blood pressure detected during a check-up does not directly diagnose sleep apnoea, but it may strengthen suspicion when symptoms such as snoring and daytime sleepiness are also present.

A single high blood pressure measurement is not sufficient to diagnose hypertension. The measurement may need to be repeated under appropriate conditions, with home or 24-hour blood pressure monitoring when necessary.

When chest pain, fainting, significant shortness of breath or a newly developed serious rhythm disturbance is present, assessment by the relevant specialty should be obtained without waiting for routine check-up results.

Do Blood Tests Show Sleep Apnoea?

Blood tests do not directly show sleep apnoea. A person may have sleep apnoea even when their complete blood count, blood glucose, thyroid hormones, liver function and kidney function are normal.

However, blood tests may help assess conditions that can cause similar symptoms or accompany sleep apnoea. Persistent fatigue, sleepiness and difficulty concentrating may not be caused only by sleep apnoea.

According to individual needs, the following tests may be considered during a check-up:

  • Complete blood count
  • Fasting blood glucose and HbA1c
  • Thyroid function tests
  • Liver and kidney function
  • Iron and ferritin
  • Vitamin B12
  • Lipid profile

The purpose of these tests is not to diagnose sleep apnoea but to assess whether anaemia, thyroid dysfunction, diabetes or another metabolic concern is present. A sleep study may still be needed when suspicion continues despite normal blood results. Additional testing, such as thyroid hormone assessment, may also be planned in some people.

Can an ECG Detect Sleep Apnoea?

An ECG evaluates the heart’s electrical activity and may help identify certain rhythm disturbances. However, a standard ECG cannot diagnose sleep apnoea.

Breathing and oxygen changes related to sleep apnoea usually occur throughout the night. A short daytime ECG may not show all rhythm changes occurring during sleep.

A physician may plan prolonged rhythm monitoring or other cardiac assessments for people with palpitations, fainting or suspected rhythm problems. These tests do not replace the diagnosis of sleep apnoea; they are used to assess accompanying cardiac concerns.

Can Oxygen Measurement Show Sleep Apnoea?

A normal single fingertip oxygen measurement does not exclude sleep apnoea. Oxygen levels may be normal during the day but fall repeatedly during sleep because of breathing interruptions.

Overnight oxygen monitoring may show fluctuations supporting suspicion of sleep apnoea. However, oxygen measurement alone cannot fully assess the type of breathing events, sleep stages or awakenings.

Overnight oxygen monitoring should therefore not be considered a definitive diagnostic method for every person. The results must be interpreted by a physician together with symptoms and other sleep-study data.

Which Check-Up Findings May Lead to a Sleep Study?

The presence of several risk signals rather than a single symptom may increase the need for a sleep study.

More detailed assessment may be planned in the following circumstances:

  • Loud and persistent snoring
  • A partner noticing breathing pauses during sleep
  • Waking while choking or gasping
  • Uncontrollable daytime sleepiness
  • Waking unrefreshed and with a headache
  • Difficult-to-control high blood pressure
  • A markedly increased neck circumference
  • Excess weight or rapid weight gain
  • Frequent night-time urination
  • Unexplained attention and memory problems
  • Dozing while driving
  • A history of heart rhythm problems or stroke
  • A family history of sleep apnoea

The presence of one of these findings does not mean that the person definitely has sleep apnoea. The degree of risk, frequency of symptoms and general health should be assessed together.

Does Everyone Who Snores Have Sleep Apnoea?

No. Snoring may occur when tissues in the upper airway vibrate during sleep, and not every person who snores has breathing interruptions.

Snoring is more concerning for sleep apnoea when it is loud and irregular and includes periods of silence. These silent periods may be followed by deep breathing, choking sounds or sudden waking.

Snoring alone does not establish a diagnosis, and the absence of snoring does not completely exclude sleep apnoea. Particularly in women, symptoms may sometimes appear as fatigue, headaches, insomnia and mood changes rather than obvious snoring.

Who Has a Higher Risk of Sleep Apnoea?

Sleep apnoea may occur at any age and in people with different body types. However, certain factors may increase risk.

The main risk factors include:

  • Excess weight and obesity
  • Increased neck circumference
  • Large tonsils
  • A narrow upper airway
  • A receding lower jaw
  • Nasal obstruction
  • Older age
  • Family history of sleep apnoea
  • Alcohol and sedative use
  • Smoking
  • The period after menopause
  • Certain cardiac, neurological and hormonal conditions
  • Opioid use

The presence of a risk factor does not mean that the person definitely has sleep apnoea. Similarly, sleep apnoea may occur in someone without obvious risk factors.

Can Sleep Apnoea Symptoms Be Different in Women?

Sleep apnoea in women may not always present with classic loud snoring and obvious breathing pauses. Fatigue, morning headaches, insomnia, frequent waking, mood changes and difficulty concentrating may be more noticeable.

These symptoms may be confused with a demanding workload, stress, menopause, anaemia or thyroid problems. The assessment should therefore not be considered complete after blood tests alone.

The possibility of sleep apnoea should be considered particularly after menopause when unrefreshing sleep and daytime sleepiness occur together with weight gain or high blood pressure.

Can Sleep Apnoea Be Identified During a Child’s Check-Up?

Sleep apnoea may cause different symptoms in children. In addition to snoring and breathing pauses during sleep, children may sleep with their mouths open, have restless sleep, sweat at night, wet the bed, struggle to wake in the morning, have difficulty concentrating or appear overactive.

Growth, tonsils, nasal breathing and changes in school performance may be reviewed during a child health assessment.

Sleep apnoea in children should not be diagnosed only according to symptoms or risk criteria used for adults. Assessment by Paediatrics, Ear, Nose and Throat specialists or professionals working in paediatric sleep medicine may be needed.

How Is Sleep Apnoea Diagnosed?

The main diagnostic method is recording breathing and sleep characteristics while the person is asleep. The physician determines which test is suitable according to symptoms, medical history and accompanying conditions.

Polysomnography

Polysomnography is a comprehensive overnight sleep study carried out in a sleep laboratory. Several sensors record data related to sleep and breathing throughout the night.

The main data that may be assessed include:

  • Brain waves and sleep stages
  • Eye movements
  • Muscle activity
  • Airflow through the nose and mouth
  • Chest and abdominal breathing movements
  • Blood oxygen levels
  • Heart rhythm
  • Body position
  • Snoring and breathing events

Polysomnography helps determine the type and severity of sleep apnoea. The results should be interpreted together with symptoms by a physician experienced in sleep medicine rather than relying only on the device report.

Home Sleep Apnoea Test

A home sleep apnoea test is a more limited breathing assessment that may be performed in the person’s own bed when considered appropriate for an adult. It usually records information such as airflow, breathing movements, pulse and oxygen levels.

A home test may not be suitable for everyone. Laboratory testing may be preferred for people with serious heart or lung disease, neurological conditions, suspected alternative sleep disorders or possible central sleep apnoea.

Laboratory polysomnography may be needed when the home test is normal or technically inadequate but suspicion of sleep apnoea remains strong. Official health information also states that sleep studies are used for diagnosis and determining the severity of the condition.

What Does the Apnoea-Hypopnoea Index Mean?

Sleep study reports frequently include a value called the Apnoea-Hypopnoea Index, or AHI. This value represents the average number of breathing pauses and reductions occurring during one hour of sleep.

Although AHI is important in assessing sleep apnoea, it does not explain the entire clinical picture on its own. The degree of oxygen reduction, symptoms, sleep duration, body position and accompanying conditions are also reviewed.

It is therefore not appropriate to make a personal diagnosis or treatment decision based on a single number in the report.

Can Smartwatches Detect Sleep Apnoea?

Smartwatches and sleep trackers may provide information about snoring, pulse, oxygen changes or sleep duration. These data may help people notice their sleep patterns and prepare for a medical consultation.

However, measurement features and accuracy vary between consumer devices. A warning that indicates “risk” does not establish a diagnosis, and a normal result does not reliably exclude sleep apnoea.

Smartwatch data may be shared with a physician as supporting information when symptoms are present. Medical assessment and an appropriate sleep study are required for diagnosis.

What May Happen if Sleep Apnoea Is Not Treated?

Sleep apnoea may repeatedly disrupt sleep throughout the night. This may be associated with waking tired, daytime sleepiness, reduced attention and impaired performance at work or school.

Excessive daytime sleepiness may create a safety risk, particularly while driving or operating machinery that requires attention. People who doze while driving should not continue driving and should obtain medical assessment without delay.

Untreated sleep apnoea is associated with serious health concerns such as high blood pressure, cardiovascular disease and stroke. Diagnosis and treatment should be planned individually.

Which Department Should You Visit for Sleep Apnoea?

The initial assessment for suspected sleep apnoea may be performed by Respiratory Medicine or a physician working in sleep medicine. An Ear, Nose and Throat assessment may also be needed when an anatomical upper-airway problem is suspected.

According to the symptoms and accompanying conditions, opinions from Cardiology, Neurology, Endocrinology, Internal Medicine or Dentistry may also be required.

New significant shortness of breath, chest pain, fainting or a serious rhythm disturbance should not be attributed only to sleep apnoea, and emergency or in-person assessment should not be delayed.

How Should Sleep Concerns Be Recorded Before a Check-Up?

Keeping a sleep diary for one or two weeks before a check-up may help symptoms be assessed more accurately. Bedtime, waking time, night-time awakenings and daytime sleepiness may be recorded.

The following information may be noted before the appointment:

  • Bedtime and waking time
  • Number of awakenings during the night
  • Whether the person feels refreshed in the morning
  • Frequency of snoring
  • Breathing pauses noticed by a partner
  • Morning headaches and dry mouth
  • Situations in which daytime dozing occurs
  • Sleep, pain or sedative medicines being used
  • Alcohol intake
  • Recent weight changes

A sleep diary does not establish a diagnosis, but it may help the physician decide which tests are needed. Monitoring sleep duration and daytime sleepiness through a sleep diary may also support the diagnostic process.
